我遇到了同样的问题，我从https://dashboard.heroku.com/apps删除了数据库（这是 Navy）添加了一个新的 PG 数据库（这是蓝色的）

From the terminal ran:
heroku config

You should see your db url
heroku config:set DATABASE_URL=postgres://your-db-url

Ran
heroku run rake db:migrate

一切顺利。我仍然不知道为什么会发生这种事！
